- Register in person at 815 South Walnut Ave, Cookeville TN. Monday thru Friday between the hours of 9am to 4pm
- You will complete a registration application and criminal background disclosure form or you can download the application.
- We will review your application. Unfortunately, we have more applicants then our class can hold.
- We will contact you personally if you have been accepted into the class.
- Payment in full is due after acceptance in the class and prior to a background check
- After your acceptance into the class we will perform a background check.
- Our CNA instructor will contact you regarding the start of the course.
